The Daviess County Health Department in partnership with Washington Community Schools will be
holding a free COVID-19 vaccine clinic for anyone 12 years of age or older. Those that had their first
dose on September 8 will have second doses at this clinic. Anyone wanting a first dose of vaccine is
welcome but will need to schedule their second dose to be administered at the Health Department.
When: October 6, 2021
1:00 pm – 3:20 pm, for students and staff.
3:30 pm – 6:00 pm, open to public.
Where: Washington Jr. High School
Make entrance at the gymnasium.

What: Moderna and Pfizer COVID-19 vaccine – 2 dose series
Anyone younger than 18 can only receive Pfizer.
Second dose will be scheduled for 21-28 days after, to be administered at the HD.
The COVID-19 shot is free to everyone in Indiana. You do NOT need insurance to get it. However,
providers can bill insurance a fee, at no cost to you, we ask that you bring your insurance card if you
have one. Your personal information is confidential. Each person will need to complete a patient
information sheet prior to receiving vaccine, even if you are receiving 2nd dose.
