The Dubois County Health Department will be holding a special pediatric vaccine clinic this evening.
The clinic will accommodate the 5–11-year-old pediatric patients and will be held at the Dubois County Health Department from 3:30 pm – 6:00 pm tonight.
Anyone 5 and older is eligible to receive the Pfizer Covid-19 vaccine. A parent needs to be present for anyone 5–18-years old.
If this is not an option, you must complete the “Consent for COVID-19 Vaccination Form” available at the Dubois County Health Department.
A copy of a parent’s ID and insurance card must be included. No appointments are necessary.
