After opening its doors in May, Crossroads Behavioral Health LLC in Huntingburg, or the old St. Joseph Hospital, is now closed after less than a year. Officials with the Indiana State Department of Health told WFIE that they planned to conduct a review of the facility, but didn’t because it lost its CMS certification.
