DCH is now a part of an innovative wellness program

 

Daviess Community Hospital has announced they are now participating in the National Rural Accountable Care Consortium.  It’s a program designed to help rural Medicare patients live, longer healthier lives. The consortium includes over 250 rural community health systems in 40 states dedicated to helping rural providers achieve the highest level of healthcare and customer service.

DCH says they will hire and train a Care Coordinator to work closely with patient.   They will help patients stay healthier and get care when needed while avoiding costly and wasteful repetitive tests.

CEO David Bixler says the hospital can help patients find the right healthcare providers, coordinate their care and help get the best results through the innovative health and wellness program.

He adds that the community can play a larger role in promoting health by taking responsibility for our own health and each other.  He says quitting smoking, eating healthier and regular exercise is all a part of living better and longer.  He urged Medicare patients to get their free Annual Wellness Visit to prevent disease and maintain health.
